Clinical Care Supervisor (LPN)
Our Heroic Story:
What began with a personal search for better care revealed a simple truth. How you age changes everything. Families needed more than support. They needed care that felt human, consistent, and worthy of trust. That belief became the foundation of Hero Home Care and a commitment to transform how care is delivered for a better aging journey.
Since then, Hero Home Care has been trusted by more than 3,000 families and has delivered over 1,000,000 hours of in-home care across its communities. Each hour represents more than service. It represents trust, relationships, and the privilege of being part of someone’s life.
At its core, Hero Home Care believes home care is about more than just care. It is about helping people stay connected, supported, and living the life they love, with a focus on helping people live more Moments That MatterTM each and every day.
Who We’re Looking For:
You are the nurse a caregiver calls when something does not feel right. You notice a subtle change, ask the right questions and turn clinical judgement into a clear next step. You also love helping others become more capable, confident and compassionate in their work.
We are looking for an experienced Licensed Practical Nurse (LPN) who wants to bring that combination of clinical skill and people leadership to Hero Home Care. As our Clinical Care Supervisor, you will bridge clinical oversight and frontline care, helping clients remain safe, independent and engaged in the comfort of their own homes.
You will be the primary clinical resource and supervisor for a designated regional team of Community Health Workers (CHWs), our frontline Heroes. You will work alongside them in clients’ homes, observe skills, provide client-specific training and delegation, coach performance and help resolve client and family concerns. Your work will make safe, personalized care possible every day.
You bring calm judgement, empathy and strong follow-through. You can assess changing conditions without direct on-site supervision, recognize when to escalate and explain what needs to happen next.
You coach with respect, set clear expectations and hold people accountable while helping them succeed.
This role calls for someone who enjoys being out in the community and staying close to the work.
Approximately 50–60% of your week is spent in the field, with the remaining 40–50% focused on documentation, care coordination and clinical administration.
You are organized, comfortable with technology and ready to take initiative as our care team grows.
A Day In The Life Of This Role:
You begin by reviewing care team updates – perhaps a client has returned from hospital, or a CHW needs guidance about medication administration. You update your shared calendar, confirm your visits and identify what needs attention first.
In the field, you may assess a client’s care needs and home environment, observe a CHW supporting a personal care routine or complete client-specific Delegation of Tasks training. You offer practical coaching, check that care follows the plan and listen closely to what the client, family and Hero are telling you.
Two brief virtual huddles keep you connected: one focuses on changing client needs and urgent escalations; the other shares company updates. You work with Scheduling, Operations and the broader Health team to align caregiver skills, client needs and continuity of care.
Your afternoon may include updating care plans and clinical records, speaking with Community Case Managers or external care providers, following up with families, investigating an incident or organizing supplies. Before finishing, you confirm the next day’s appointments and hand over pressing after-hours care items to the designated call team member.
You report to the Care Operations Manager. Your hybrid week includes field visits, home-based administration and at least one day in the North Vancouver or Abbotsford office. You work 40 hours over five days during standard business hours, rotating every three months between Tuesday–Saturday and Sunday–Thursday.
Having your own car and driving throughout Vancouver, Richmond and the North Shore is essential!
What You Are Responsible For:
Clinical Assessment and Care Planning
- Perform in-home nursing assessments for new and existing clients, identifying care needs, environmental safety concerns and clinical risks. Recognize subtle changes in health and coordinate appropriate changes to care plans, interventions and schedules within your scope of practice.
- Support smooth hospital discharges and other care transitions through clear communication with families, Community Case Managers and external providers.
- Complete all care plans within 48 hours of initial intake; review and update every client’s plan at least every three months and whenever their condition changes. Investigate and document incidents and clinical escalation responses within 24–48 hours, with urgent concerns addressed promptly.
CHW Supervision, Delegation and Mentorship
- Provide real-time clinical guidance and problem-solving support. Conduct client-specific Delegation of Tasks in accordance with VCH Home Support Guidelines and applicable BCCNM standards, within your authorized scope of practice.
- Evaluate competence through structured skills observations, direct coaching and regular performance reviews. Focus on safe lifts and transfers, medication administration, infection control and consistent care plan adherence.
- Spend 50–60% of weekly hours in the field, completing 10–20 CHW field visits per week and at least one field observation or check-in per CHW every 90 days. Work towards mandatory skills-check pass rates as designated in your scorecard.
Service Quality, Safety and Governance:
- Monitor care delivery against applicable provincial requirements, occupational health and safety standards and Hero Home Care quality policies. Maintain accurate, timely records in the electronic care system.
- Oversee incident reporting, adverse-event investigation and root-cause analysis. Participate in quality audits, client satisfaction reviews and clinical policy updates, using findings to improve care and strengthen trust with families.
- Ensure every client receives a care plan review at least every 90 days and contribute to the team’s client experience and Net Promoter Score goals.
Cross-Functional Collaboration and Clinical Advocacy
- Partner with Scheduling and Operations on thoughtful client–caregiver matching, care continuity and safe alignment of skills. Review and update CHW skills-matching criteria and profile notes every 90 days.
- Represent Hero Home Care professionally with community partners, healthcare teams and funding agencies. Support clinical decisions during operational escalations and respond to Community Case Manager inquiries within 24 hours.
- Bring the perspectives of clients, families and CHWs into team decisions. Help create a supported, engaged care team and contribute to CHW experience and Net Promoter Score goals.
Risk Management, Infection Control and Emergency Response
- Complete a risk screen for every client. Identify and mitigate environmental, medical and behavioural risks that could affect clients or frontline CHWs.
- Lead regional infection prevention and control practices, including routine audits, respiratory or gastrointestinal outbreak management, personal protective equipment training and exposure reporting.
- Provide structured clinical guidance and timely triage during acute deterioration, medical emergencies and environmental crises such as extreme weather, power outages or home safety hazards. Escalate appropriately and coordinate clear handoffs for ongoing or after-hours support.
- Assess workplace safety and ergonomics, including safe transfer space and mechanical lift clearance, to help reduce caregiver injuries.
- Investigate adverse events, near-misses, falls and medication errors; implement immediate corrective actions and follow through to prevent recurrence. Work toward closing incident reports within 48 hours, coordinating follow-up where an investigation requires more time.
- Monitor medication errors and use the findings to improve safety.
Experience Required:
To be successful in this role, you must have:
- Registration in good standing with the BC College of Nurses and Midwives as a Licensed Practical Nurse, plus active Basic Life Support or CPR certification at the healthcare provider level.
- At least three years of home and community care experience and at least two years of clinical experience in a hospital or acute-care setting.
- Proven experience leading, coaching, training and developing Community Health Workers, with strong verbal and written communication skills.
- Strong clinical assessment skills in chronic disease management, wound care, medication administration, fall prevention and mobility support.
- Hands-on proficiency with home healthcare equipment, including mechanical lifts, oxygen concentrators, catheter care and tube-feeding equipment, within your professional scope.
- Confidence using electronic medical records, shared digital calendars and standard office technology, with the ability to learn new systems and help others use them effectively.
- A valid driver’s licence and access to a reliable vehicle for required community travel IS A MUST HAVE for this role.
You are an empathetic listener who can educate families and collaborate with other healthcare professionals. You organize and prioritize your own work and support the priorities of others, managing visits, supplies, deadlines and documentation without losing sight of the people behind each task.
You remain steady when priorities shift, use sound judgement with incomplete information and know when to seek support. You take ownership, close communication loops and step in when something needs attention. You are a self-starter who welcomes feedback and invests in your ongoing learning and professional growth.
